Kaiser Health News has a feature called "Bill of the Month." This one is a doozy. Even if you have health insurance from an employer, you can still get a nasty financial surprise as a consequence of a practice called balance billing.

My wife underwent a biopsy where the anesthesiologist was not in plan. She was billed for the amount he charged and she refused to pay it and they removed the charge.

But it should not get to that stage. We are asked to sign a paper before surgery that we will pay for anything not covered. Law should make that consent include choices including one choice that does not allow providers not in plan.
